Fire House

Fire House is my favourite burger shop in Japan - they sell apple hamburgers, which comes with slices of apple stacked on top of your hamburger patty. So good, no notes. Although Fire House is in a pretty central part of Tokyo, it's located in Bunkyo-ku which is probably not high on your list of places to visit if you're coming as a tourist. The Tokyo Dome is nearby, though!
